It’s easy, on your next visit to one of our Service Locations ask the front desk staff to enroll you in Heartwell’s Patient Portal and provide them with your email address. The practice provides you with an eight-digit token number to access your Patient Portal account. In turn, the practice will send you an email that contains a link to the Patient Portal Web site. Follow this link within your email or use the link below. Review the Terms and Conditions as well as enter your security token number received from your practice and your email address. Your account will be active once you follow all prompts for enrollment.
